INSURANCE: We accept all insurances as long as you can go to any dentist.
We are providers on Blue Cross/Blue Shield, Cigna, Delta Dental Plan and Delta USA. If you are not sure how your insurance
works, you can ask your human resources person at your work or call your insurance company and ask them how to go about locating
a dentist. There is usually a customer service number on the back of your insurance card. We do not except any Tenn Care or
state funded insurance companies.

SELF PAY PATIENTS: We will always let you know prior to every visit how much
your treatment will cost. We do not carry accounts, but we do accept all credit cards (Visa, MasterCard,
Discover, American Express and debit cards). We also offer interest free financing through Care Credit for anyone who
needs to extend out their payments. We offer several treatment plans to help you chose which one fits your needs and budget.

CANCELLATIONS: We expect to receive at least a 24 hour notice on any cancelled
appointments unless some emergency occurs which prevents you from keeping your appointment. We try to be on time for every
appointment and expect you to do the same. If by some chance you are 15 to 20 minutes late for your appointment, the appointment
may have to be rescheduled.

EMERGENCY PATIENTS: We accept any patient who needs emergency care. We try to
work them into our busy schedule as soon as possible. We do not accept walk-ins. All appointments must be made by phone or
in person. Our objective is to give our patients the proper care and medicine to calm their symptoms until the chosen treatment
is completed.
